Items guide hub

Items are temporary run resources. They can restore Health or Stamina, improve extraction, create an escape, attract attention, or act as currency. The right item is not the rarest icon; it is the one that changes the next dangerous decision. Because inventory space is limited, carrying a weak effect too long can be as costly as using a strong effect too early.

Coverage status — 2026-08-08: current independent and community-maintained indexes agree on the main item families and common examples. Exact effects, durations, rarity, spawn weights, shop prices, event availability, and discount interactions are community data and need a live check. This is a currently documented decision index, not a claim of an immutable complete count.

Item families

Distraction items such as Air Horn deliberately create attention or noise. They belong in a planned route with a capable user. Currency items include Research capsules and Tapes in community categorization, but their operation differs: capsules advance a named Twisted’s Research and provide account value, while Tapes exist for the current run.

Healing decisions

Bandage is currently documented as restoring one Heart, while Health Kit restores to maximum Health. That makes current Health the first question. Do not spend a full-heal item for one missing Heart when a Bandage is available and the run remains safe.

End the chase before healing. An animation or inventory interaction in direct danger can trade the restored Heart immediately. Communicate with healers and item holders; a two-Heart specialist on one Heart may deserve priority over a durable Toon in a safe role.

Extraction decisions

Instructions provides a timed extraction boost in current guides, Jumper Cable adds immediate progress, and Valve is described as completing the active machine. The strongest effect should go to a station whose completion matters: a dangerous last machine, a room becoming unsafe, or a deep floor where seconds reduce team exposure.

Do not activate a machine-only item before interacting or after leaving the station. Read the live prompt. If Twisted Astro is present, check consistency matters as much as speed because a failure can remove progress.

Escape decisions

Speed Candy helps create immediate movement; stamina items refill or improve the resource that powers sprinting; Smoke Bomb is documented as breaking ordinary Twisted interest briefly; Eject Button combines an emergency movement/stealth effect in current references. These are different points in a chase.

Use speed before a dangerous gap, stamina before the bar empties, and an emergency break when ordinary cover is not reachable. Do not wait until a hit animation removes the chance. Lethal Twisteds and special exceptions may ignore or outlast an effect, so never treat an item as invulnerability.

Tapes and Dandy’s Elevator Shop

Tapes are collected and spent during a run. Current guides document floor pickups, machines, floor survival, abilities, encounters, and item use as sources, with exact yields varying by action and patch. Keep enough currency for the floor where a shop purchase meaningfully changes survival.

At the elevator shop, scan all choices before buying. Healing has priority when a player is injured and no safer source exists. Escape helps a threatened slow Toon. Extraction helps a stable team move faster. In a coordinated Dandy run, the team may deliberately skip purchases for Twisted Dandy Research; that choice needs consent.

Inventory order and communication

Know which slot contains the emergency item. Dropping or cycling the wrong object during a chase wastes the opportunity. Use ordinary candies before rare emergency tools when the smaller effect is sufficient.

Call healing and rare items by location. Do not pick up everything automatically if a teammate needs it more. Research capsules are described as client-side, so sharing their location does not create the same competition as a single floor item.

Event and removed items

Community indexes include seasonal currencies and items, plus content no longer available. Their presence in a historical list does not prove current access. Only list an event object as obtainable when the live event or an official current notice supports it.

Where do items come from?

Current guides document floor spawns, a pre-run shop, Dandy’s Elevator Shop, Toon abilities, and special events. Source and availability vary by item.

What should I carry?

One item for the likely job and one for the likely failure is a strong baseline: extraction plus escape, or healing plus stamina. Inventory size and Toon abilities can change the answer.

Should I save rare items forever?

No. Use them when their effect prevents a hit, completes a dangerous machine, or preserves a deep run. Saving through the defeat produces no value.

Are shop prices fixed?

No. Prices and discount effects are update-sensitive. Read the live card before spending Tapes.
